Our home daycare application has been years in editing. We’ve tried multiple versions, multiple methods, and skipping this part all together, and in the end THIS 4-page PDF version is the absolute BEST way to help us decide if a family is a good fit for our care – BEFORE we do the interview.

Generally we move straight to sending initial contacts our application.
PROVIDER TIP: We don’t offer the application on our website – we’ve found that families that are actually interested will take the time to fill it out and return it AFTER the initial contact. Many families that are just poking around to see what’s out there don’t make it to this step.

Page one of our application provides basic information. Parent/Guardian info and contacts, and medical information and contacts. This is also content that the state (Illinois) requires us to keep on file for each enrolled child, so we can add this form to our enrollment paperwork if we end up enrolling the child.
PROVIDER TIP: We ask about allergies in this section and require a medical allergy plan to be in-place prior to enrollment.
Page two on our home daycare application is all about the child’s medical history and the household. The Medical History section includes questions about any/all medications, restrictions, and even potty training. The Household section includes listing all members of the child’s household, pets, and previous childcare experiences.
PROVIDER TIP: We use this section to understand not only where the child is coming from, but also what kind of support system is in place for the child and for back-up care.
Child Information is page three of our home daycare application. This section includes very specific information about the child’s current habits: nap times, feeding times, favorite toys, and comfort items to name a few. The final third of this section is our specially developed child personality description questions. With six sets of statements to choose from, parents will reveal their child’s innate qualities.
PROVIDER TIP: We use this section not only to see if the child would be a good fit with our current group, but also to find out more about what the parents will expect from childcare.
The final page of our application is the Family Practices section. This section we have worked especially hard on. As many home daycare providers know, families can be the most challenging part of the job. In this section we ask families to prioritize statements that reflect the value they place on different aspects of child-rearing, as well as asking them to choose between statements that define their goals for their child.
